The granulation tissue produced in the second phase becomes fibrous tissue in the capsule, tendons and inflamed periarticular tissue, which produces great deformation of the joint. The disappearance of articular cartilage and fibrosis of the joint space lead to joint immobilization (ankylosis). In this stage, the burst deformations of the fingers of the hands are characteristic.

Among the extra-articular manifestations visible in this phase are the presence of rheumatoid nodules in the subcutaneous and periarticular tissue in 20 to 25% of cases, and less frequently Flekosteel, in viscera.
